What Are the Causes?

The causative factors which result in dyspareunia are typically of musculoskeletal origin. This can stem from low back pain, hip pathology, sacroiliac dysfunctions, bladder dysfunctions such as interstitial cystitis, endometriosis, autoimmune diseases, perineal tears, spine dysfunctions, and chronic pain syndromes. Therefore, this is not merely an emotional reaction but a physical issue that can be resolved and treated like most musculoskeletal dysfunctions. 

What Are the Symptoms?

Pain with intercourse can vary from patient to patient. For some, even the attempt at intercourse is impossible, and for others, it is possible but uncomfortable. This pain can be described in a variety of levels:

  • Burning
  • Feeling Like Something Is Blocking the Entrance
  • Extreme Sensitivity
  • Tearing
  • Deep Pain or Superficial Pain
  • Abdominal/Pelvic Pain
  • Cramping
  • Tailbone Pain

What Can I Expect in PT?

Initially, you will discuss your symptoms, past medical history, and your goals with your therapist. According to your information and medical history, a musculoskeletal examination will be performed to evaluate any spine or extremity dysfunctions that may be contributing to your painful intercourse. Once this information is obtained from the orthopedic evaluation, the next step is a pelvic floor examination. We know this can be intimidating, so your comfort is our utmost concern through your evaluation. Upon your consent, an examination can be performed to evaluate your pelvic floor. This examination is performed to determine the high or low muscle tone, connective tissue restrictions, symmetry or asymmetry of the pelvic floor, hypersensitivity issues, and the pelvic floor's musculoskeletal integrity. All of the information gained from your initial appointment and your goals will be used to map a plan of care to resolve your issues.
